- Identifying health problems is the second phase in the process of planning, delivering and evaluating personcentred care. A health problem is identified as a result of synthesising, analysing and interpreting the data collected during the assessment phase. A health problem is a condition that necessitates intervention to prevent or resolve disease or illness or to promote coping and wellness. This chapter will build on the assessment phase outlined in the previous chapter. This process of identifying health problems is explored throughout the chapter in the context of person-centred care.
